To begin, you must retrieve your official criminal history reports, which require **Fingerprints, Forms, and Fees.** We need two separate reports to ensure a complete record seal: your State of Nevada CHR and your local SCOPE.

Requirement 1: Nevada CHR ($27 Fee)

  • Nevada Criminal History Repository (CHR) Report:
  • You must submit a new, original **FD-258 fingerprint card** to the Department of Public Safety.
  • The mandatory state processing fee for this report is $27.00 (**must be a Money Order**).
  • Fill out the CHR Request form (DPS-006).

  • Local SCOPE Report:
  • To retrieve your local arrest history (SCOPE) from the Metropolitan Police Department, a separate processing fee of $13.00 is required.

Step 2: Complete Our Client Document and Obtain Notary

Step 3: Mail Your Complete Packet

Send the following complete packet to the Real Records office for review and filing. Consolidating all materials ensures the fastest possible processing time.

Checklist of Required Items:

  • Local SCOPE Report
  • Your original **FD-258 Fingerprint Card** (for your CHR request).
  • Original Money Order for CHR ($27).
  • Filled out and notarized Authorization Form.
  • Filled out and ink-signed CHR Request form (DPS-006).
  • Send JOC (Judgment of Conviction) if not yet sent.
  • Send copy of ID if not yet sent.
